Kamen Rider Knight Survive (Kamen Rider Ryuki)
As the series progressed, Ren Akiyama was given the Survive Shippu Advent Card. With the power of wind, he was able to turn his Dark Visor into the Dark Visor-Zwei. Once inserting the card into it, he was able to assume his Survive form. Once assuming this form, his personal Advent Beast, Darkwing, has now evolved into Darkriader, where it can now assume a bike form.

Kamen Rider Garren Jack Form (Kamen Rider Blade)
After sealing the Peacock Undead, he was then given his own Rouse Absorber. By using the Absorb Serpent and Fusion Jack cards, he is able to his Jack Form.

Kamen Rider Gatack Hyper Form (Kamen Rider Kabuto)
Only seen in the Hyper Battle DVD, Arata Kagami, aka Kamen Rider Gatack, used a Hyper Zecter from the future to assume his own Hyper Form. This was pretty much the plot of that special where Arata was trying to act like Tendou.

Kamen Rider Zeronos Zero Form (Kamen Rider Den-O)
In Episode 40, to combat the Death Imagin, Yuto uses the last of his Red Zeronos Cards to assume this form. By using it the first time, it used his memories as a catalyst of some sort to make his green portions turn to rust. In this form, he can use the Denebick Buster to fight.

Kamen Rider Rising IXA (Kamen Rider Kiva)
When Keisuke was relieved of his duties as IXA, Kengo was given the Ixa System, but had to resign, pretended that he was flawed. Keisuke was given back the Ixa Driver and was able to use it's upgraded system. By using the Ixariser, he was able to transform into Rising Ixa.

Kamen Rider Diend Complete Form (Kamen Rider Decade)
Used in Episode Yellow, Kaito was given his own K-Touch. By using it, he was able to assume his Complete Form, which is comprised of the cards of movie-exclusive Riders, like G4, Ryuga, Orga, Glaive, Kabuki, Caucasus, Arc, and Skull.

In the V-Cinema, Kamen Rider Accel, by comining his Accel Gaia Memory with the Gaia Memory Enhancing Adapter, Ryu Terui was able to assume Booster Form.

Kamen Rider Birth Day (Kamen Rider OOO)
After Akira Date gave left, his friend, Shintaro Goto assumed the mantle of Kamen Rider Birth. To assume his final form, he uses all of the Birth CLAWs weapons to assume Birth Day form.

Kamen Rider Meteor Storm (Kamen Rider Fourze)
In Episode 28, Ryusei uses the Meteor Storm Switch to assume this form. With it, he can use the Meteor Storm Shaft, which is a long boe he can use for combat. He can insert the switch into the shaft and spin it like a BeyBlade (Imagine all the tweets that made BeyBlade jokes when this form was first announced).

Kamen Rider Beast Hyper (Kamen Rider Fourze)
In Episode 29, Kousuke finds the Mirage Magnum. By using the Beast Hyper Ring into his belt, he can transform into his Hyper Form. In this form, he can use the Mirage Magnum as his weapon of choice. (Oh and by the way, congratulations to Tasaku Nagase on his marriage).

Over Lord Inves Baron (Kamen Rider Gaim)
Many can argue if this is Baron's final form or not, as others will say that it is his Lemon Energy Arms. To be honest, this should be his final form, regardless that he is not a Kamen Rider in this state. The reason why I'm saying this is his final form is because it was treated as something powerful. Kaito had equal power to Kouta when the latter assumed Kiwami Arms. Anyways, how did he assume this form? After being beaten in a fight against Ryoma Sengoku (aka Kamen Rider Duke), he eats an Inves fruit and transforms into an Overlord. With this power, he is able to kill Ryoma. With this kind of power, he goes down the dark path, where he would eventually meet his fate at the hands of Kouta.

Kamen Rider Mach Chaser (Kamen Rider Drive)
In the V-Cinema, Kamen Rider Drive Saga: Kamen Rider Mach, Go Shijima is able to use the Ride Crosser Shift Car to transform into Kamen Rider Mach Chaser, a hybrid form combining the powers of both Mach and Chaser.

Kamen Rider Sin Specter (Kamen Rider Ghost)
In his V-Cinema, Kamen Rider Ghost RE:BIRTH: Kamen Rider Specter, Makoto Fukami uses the Sin Specter Eyecon to assume Sin Specter Damashii, which is obviously a repaint of Mugen Damashii, but regardless, I love this form.

Kamen Rider Brave Taddle Legacy Gamer Level 100 (Kamen Rider Ex-Aid)
In Episode 37, Hiiro Kagami was given the Taddle Legacy Gashat and was able to assume Level 100, Taddle Legacy Gamer. This was at the time where Hiiro was working for Masamune Dan (aka Kamen Rider Chronos) so he could get the data of his deceased girlfriend, Saki Momose. This form was also used by his Game World counterpart, who assumed the mantle of Kamen Rider True Brave.

Kamen Rider Cross-Z Magma (Kamen Rider Build)
In Episode 31, after it was revealed that Ryuga was half-alien, half-human, he uses his powers to give birth to the Dragon Magma Fullbottle. By inserting it into the Cross-Z Magma Knuckle, he can transform into Kamen Rider Cross-Z Magma, which is revolved around lava.
